Tooth Taxi Coming to Seaside High School

The tooth taxi will be coming to Seaside High School during the week of May 8-12. Students can receive dental care regardless of whether they have dental insurance. Students cannot receive dental care, however, unless the required paperwork has been completed. Students who have private insurance (i.e. Blue Cross, Moda, etc.) must also provide a copy of both sides of their insurance card. Students on OHP do not need to provide copies of their insurance documents. Students without insurance are welcomed to apply for care.

Copies of the required Tooth Taxi forms are located in the High School office. You can also download the forms from this website. Please take the completed forms and insurance documents, if needed, to the Seaside High School office as soon as possible.

El taxi dental vendrá a Seaside High School durante la semana del 8 al 12 de mayo. Los estudiantes pueden recibir atención dental independientemente de si tienen seguro dental.  School Wellness Policies are required by the U.S. Department of Agriculture from school districts receiving federally-funded school meals. These School Wellness Policies set goals for school-based activities that promote healthy eating, daily physical activity, and other wellness behaviors. Individual schools must implement the district policy but can also go above and beyond the policy to improve student health.
